The Format Document button is an ActiveX control embedded in the document. ActiveX must be enabled for it to appear and respond.
•Microsoft Word > Menu File > Options > Trust Center
•In Trust Center Settings…, select ActiveX Settings
•Select Prompt me before enabling all controls with minimal restrictions (recommended, this is the default)
•Keep Safe mode checked
•Click OK
